Authorization to Electronically File Individual Tax Return
I authorize Hyun J Ju, CPA & Associates APC to electronically file my federal and/or state individual income tax return for the tax year indicated above with the Internal Revenue Service and applicable state tax authority.
By signing below, I confirm that I have reviewed the individual tax return(s) to be filed and the amounts shown are accurate and complete to the best of my knowledge.
I understand that this authorization is valid for the current tax filing season only and must be renewed annually.
I acknowledge that any refund will be issued in the manner indicated on the return, and that I am responsible for any taxes, penalties, or interest that may be due.

Authorization to Electronically File Business Tax Return
As an authorized officer/member of the above-named business entity, I authorize Hyun J Ju, CPA & Associates APC to electronically file the federal and/or state business tax return (including but not limited to Forms 1120, 1120-S, 1065, or applicable state equivalents) for the tax year indicated above.
By signing below, I confirm that I am duly authorized to sign on behalf of the business entity and that I have reviewed the business tax return(s) to be filed. The information provided is accurate and complete to the best of my knowledge.
I understand that this authorization is valid for the current tax filing season only and must be renewed annually.
I acknowledge that the business entity is responsible for any taxes, penalties, or interest that may be due.
Privacy & Confidentiality Consent
Information We Collect
We collect personal and financial information necessary to provide tax and accounting services, including: name, address, Social Security Number/EIN, income information, deductions, and financial statements.
How We Use Your Information
Your information is used solely for the purpose of preparing your tax returns and providing related accounting services. We do not sell, rent, or share your personal information with third parties except as required by law.
Data Security
We maintain strict security procedures to protect your information, including encrypted digital storage and secure communication channels.
Retention Policy
Client records are retained for a minimum of 7 years in accordance with IRS guidelines and professional standards.
Engagement Letter
Scope of Services
Hyun J Ju, CPA & Associates APC agrees to provide: preparation of federal and state individual income tax returns, tax planning advice, and related accounting services as mutually agreed upon.
Client Responsibilities
The client agrees to provide all necessary financial information and documentation in a timely manner, respond promptly to requests for additional information, and review all completed tax returns before filing.
Fees
Our fees are based on the complexity of the work performed and will be communicated prior to commencement of services. Payment is due upon completion.
Limitation of Liability
The Firm's liability for any errors or omissions shall be limited to the fees paid for the specific service in question.
Confidentiality
The Firm will maintain strict confidentiality of all client information in accordance with the AICPA Code of Professional Conduct and applicable state laws.
ACH Transaction Authorization
ACH Payment Authorization / ACH 결제 승인
1. Tax Refund & Payment via ACH (세금 환급 및 납부)
I authorize Hyun J Ju, CPA & Associates APC ("the Firm") to initiate ACH (Automated Clearing House) electronic transactions on my behalf for the purpose of processing federal and/or state tax refunds and tax payments. This includes direct deposit of tax refunds into my designated bank account and electronic payment of taxes owed to the IRS and/or state tax agencies (e.g., California Franchise Tax Board).
본인은 Hyun J Ju, CPA & Associates APC("본 사무소")가 연방 및/또는 주 세금 환급 처리와 세금 납부를 위해 본인을 대신하여 ACH(자동 청산소) 전자 거래를 개시하는 것을 승인합니다. 여기에는 세금 환급금을 본인의 지정된 은행 계좌로 직접 입금하는 것과 IRS 및/또는 주 세무 당국(예: California Franchise Tax Board)에 대한 전자 세금 납부가 포함됩니다.
2. CPA Service Fee Auto-Debit via ACH (CPA 수수료 자동 이체)
I authorize the Firm to initiate ACH debit entries from my designated bank account for payment of professional service fees. The fee amount will be communicated to me in writing (via email or invoice) prior to each debit transaction. No ACH debit will be initiated without prior written notification of the amount due.
본인은 전문 서비스 수수료 결제를 위해 본 사무소가 본인의 지정된 은행 계좌에서 ACH 인출 거래를 개시하는 것을 승인합니다. 수수료 금액은 각 인출 거래 전에 서면(이메일 또는 인보이스)으로 사전 통보됩니다. 사전 서면 통보 없이는 ACH 인출이 실행되지 않습니다.
3. Right to Cancel (취소 권리)
This authorization shall remain in effect until I notify the Firm in writing (email to hju@hjucpa.com or written letter) that I wish to revoke this authorization. I understand that I have the right to cancel this authorization at any time by providing written notice at least 3 business days prior to the next scheduled transaction. I also have the right to stop payment by contacting my financial institution.
본 승인은 본인이 본 사무소에 서면(hju@hjucpa.com 이메일 또는 서면 서신)으로 본 승인을 철회하겠다는 통보를 할 때까지 유효합니다. 본인은 다음 예정된 거래 최소 3영업일 전에 서면 통보를 제공하여 언제든지 본 승인을 취소할 권리가 있음을 이해합니다. 또한 본인의 금융 기관에 연락하여 결제를 중단할 권리가 있습니다.
4. Liability & Dispute (책임 및 분쟁)
If any ACH debit is returned due to insufficient funds or any other reason, the Firm reserves the right to collect the amount due by other means. I understand that if an erroneous debit occurs, the Firm will correct the error within a reasonable timeframe. Any disputes should be directed to the Firm at hju@hjucpa.com within 60 days of the transaction date.
잔액 부족 또는 기타 사유로 ACH 인출이 반환될 경우, 본 사무소는 다른 방법으로 미지급 금액을 회수할 권리를 보유합니다. 잘못된 인출이 발생한 경우 본 사무소는 합리적인 기간 내에 오류를 수정합니다. 모든 분쟁은 거래일로부터 60일 이내에 hju@hjucpa.com으로 본 사무소에 알려주시기 바랍니다.
5. Record Retention (기록 보관)
A copy of this authorization will be retained by the Firm for a minimum of two (2) years from the date of the last ACH transaction, in accordance with NACHA Operating Rules.
본 승인서의 사본은 NACHA 운영 규칙에 따라 마지막 ACH 거래일로부터 최소 2년간 본 사무소에 보관됩니다.
